Blood Fat Patterns and Iron Markers Linked to Anemia Severity in Dialysis Patients

This study looked at fatty acids in red blood cell membranes, the omega-3 index, and a marker called zinc protoporphyrin (ZnPP) in 117 people undergoing hemodialysis, compared with 28 healthy people. ZnPP is a marker that rises when the body doesn’t have enough iron to make hemoglobin efficiently.

Dialysis patients had a much lower omega-3 index than healthy controls, but the omega-3 index itself didn’t differ between patients with milder versus more severe anemia. However, certain other fatty acids did track with anemia severity: patients with lower hemoglobin tended to have higher levels of palmitic acid and erucic acid, and lower levels of oleic acid in their red blood cell membranes. After statistical correction for multiple comparisons, only the link between oleic acid and hemoglobin remained significant.

Separately, ZnPP levels rose steadily as hemoglobin dropped, and this relationship held up even after adjusting for other factors, suggesting ZnPP could serve as a continuous indicator of anemia severity and iron-limited red blood cell production in dialysis patients.
